About
Exeter City Primary Care Network (PCN) brings together six local GP practices in the centre of Exeter, collectively supporting around 39,000 patients. The PCN enables partnering practices to work collaboratively at scale and provide their patients with access to additional services. This neighbourhood approach to care is guided by the principle that each practice best understands its patients' needs and will work together to foster collaboration, deliver services, and enhance care for the broader population.
These services include the Same Day Physio and Mental Health Service, Care Home Ward Round Team, Women’s Menopause Support Service, Clinical Pharmacy Support, Wellbeing Coaches, Nutritional and Diabetes Workshops, Cancer Care Services, and coordinators supporting patient education, health service navigation, and signposting.
